Supported Independent Living
We provide supported independent living services to support our clients around the clock, 7 days a week. SIL services can include 24/7 daily living support, help to manage money and household budgeting, and provide personal care such as showering and dressing. Other services include supporting household participation and community integration, and assisting with cooking. cleaning, and laundry, providing support for grocery shopping and catching public transport, and supporting with creating new social networks and visiting family.

Epiphany Care offers dedicated support coordination services to assist you with navigating the NDIS and what you are entitled to.

Social and Community Participation
Our experienced staff supports you in getting you and about in your communities so you can be part of activities that are important to you. We provide you with access to community events, social gatherings assistance with grocery shopping as well as travel and transport.

Day Programs
Our Disability Day Programs are hubs where programs and activities are specifically designed for people with disability to learn practical life skills while building on confidence and social skills in an encouraging, supportive environment.
At our hubs you can learn something new like cooking or how to use a computer. You can also go on day trips to local events, swimming pools, gardens, museums and much more.
Our support workers will accompany you to activities, whether it`s shopping, playing sport, or just hanging out in a cafe.

Respite Care and STA
We offer a broad range of respite accommodation, care, and support services that aim to integrate the community and give our participants the highest quality of care available. Our respite support services are tailored to individual participant requirements.

Travel and Transport
Epiphany Care supports travel and transport to help enable participation in the community, social economic, and daily life activities. We have vehicles that are wheelchair accessible and friendly to allow participants to expand their horizons. Our staff aims to provide safe, prompt, and responsible travel arrangements to help you get to and travel arrangements to help you get to and from work, shopping, and appointments.

Assistance with Daily life & Domestic Assistance
Epiphany team can assist your personal care needs with respect and dignity. These include bathing, showering, maintaining, personal hygiene, grooming, dressing, toileting mobility support, and getting in and out of bed. We can provide help and assistance with different tasks around the home and domestic activities so that you can have a safe, secure, and healthy environment.

Group/Centre Based Activities
Epiphany Care Community Hubs offer an inclusive environment and the opportunity for learning, friendship, growth and most importantly, fun! Enjoy a wide range of social experiences and activities including sports, arts and crafts, music and special activities such as trivia, disco and day trips.
